Service Provider Affiliation

If you are interested in providing developmental disability services in Coffey, Franklin, or Osage Counties, your must affiliate with the ECKAAA CDDO to bill Medicaid for those services.  The list of required affiliation documents can be downloaded below.  Please contact CDDO Coordinator: Vicki Seems vickis@eckaaa.org to request an affiliate agreement.
