Our mission is to educate, counsel, and provide supportive services. We believe that you deserve to be supported, informed, and connected, and we're dedicated to providing the resources you need to navigate your journey. The agency provides some services directly and contracts for others.
Agency services include:
- Information and Assistance: Connecting you to the right agency and community resources and services.
- Nutrition Services: we provide access to both congregate and home delivered meal services. (see definitions below)
- Congregate Meals: Nutritious meals are served in a group setting at one of our nutrition sites or through the voucher program. Congregate dining offers opportunities for socialization and fellowship in local communities.
- Home-Delivered Meals: Nutritious meals are delivered directly to the home for those who are homebound and meet program eligibility.
- Case Management: Assistance in access and care coordination of information and services to support older adults in the living environment of their choice.
- Homemaker Services: Providing assistance with things such as light housekeeping, laundry, and meal preparation.
- Attendant Care Services: Assistance with personal care such as bathing, dressing, toileting, and home medication.
- Caregiver Resources, Information and Services: Gain access to helpful information, to information to support you as a caregiver, and to support groups.
- Respite Care Services: Temporary respite care to provide caregivers with short-term relief through federal or state funded programs.
- Medical Equipment: Case Management clients are provided assistance in accessing medical equipment as agency program guidelines allows.
- Grandparent Resources: Support and information specific to grandparents raising grandchildren.
- Senior Health Insurance Counseling (SHICK), Medicare Consultation: Unbiased assistance in understanding and navigating Medicare and supplemental insurance.
- Legal Assistance: Access to legal guidance for things like wills, power of attorney and estate planning.
- Health and Wellness Programs: The agency sponsors and provides access to evidence based health and exercise programs and opportunities in local communities.
- Caregiver Support Groups: Connect with other caregivers for emotional support and shared experiences.
- Intergeneration Programs: Fostering connections and enriching lives across generations.
- Linkage to other services: The agency can help you and your family locate and access other services the agency does not provide directly.
- Volunteer Opportunities: Giving back to the community and connecting with others through volunteering at your local nutrition site: Preparing, packing, and delivering meals.
