PACE Program

For seniors seeking to maintain their independence and stay in their homes, the Program of All-Around* Care for the Elderly (PACE) offers a valuable and comprehensive solution. Now available in Franklin, Miami, and Osage Counties,

PACE provides coordinated care and support services designed to help older adults age gracefully in their communities.


What is PACE?

PACE is a unique, person-centered program that integrates medical, social, and long-term care services. It's a holistic approach that recognizes the unique needs of seniors and aims to:

  • Promote Independence: Empowering participants to live comfortably and safely in their own homes for as long as possible.
  • Provide Comprehensive Care: Offering a wide range of medical and social services, tailored to individual needs and preferences.
  • Reduce Hospitalizations: Proactively managing health conditions and providing preventative care to avoid unnecessary hospital visits.
  • Support Caregivers: Offering respite and assistance to family members who are often the primary caregivers for their loved ones.

Who is Eligible for PACE?

To be eligible for PACE, individuals typically need to meet the following criteria:

  • Age 55 or older
  • Live in Franklin, Miami, or Osage County
  • Certified by the State of Kansas as needing a nursing home level of care
  • Able to live safely in the community with PACE support.

What Services Does PACE Offer?

PACE offers a wide array of services designed to address the specific needs of participants. These services may include, but are not limited to:

  • Primary Care: Medical services provided by PACE physicians and nurses.
  • Specialty Care: Access to specialists such as cardiologists, neurologist, and therapists.
  • Prescription Drugs: Management and delivery of medications.
  • Adult Day Care:  Socialization, recreation, and therapeutic activities in a supervised setting.
  • Rehabilitation Services: Physical, occupational, and speech therapy.
  • Home Care: Assistance with personal care, homemaking, and errands.
  • Transportation: To and from the PACE center and medical appointments.
  • Nutritional Services: Meals and dietary counseling.
  • Social Services: Counseling, support groups, and assistance with accessing community resources.
  • Dental, Vision, and Hearing Care
  • Hospice Care:  When needed, providing compassionate end-of-life care.
